Form AROCC-365-4 is a 4 part carbon-less automotive SMOG & REPAIR work order & invoice – 11″ X 8.5″. This form is popular because it can also serve as a Smog Check (emissions) testing work order. AROCC-365-4 includes the smog check facility disclaimer that states: “BY LAW, YOU MAY CHOOSE ANOTHER LICENSED SMOG CHECK FACILITY TO PERFORM ANY NEEDED REPAIRS OF ADJUSTMENTS THAT THE SMOG CHECK TEST INDICATES ARE NECESSARY. A CUSTOMER WILL BE CHARGED FOR INSPECTION REGARDLESS OF PASS OR FAIL”.

A free imprint of your company and address is included as well as a free logo that would print to the left or right of your company info. The logo will print the same color as the imprint text.
